I hereby declare that I have voluntarily enrolled in the sports training(s) / competition(s) offered by Sittruli Foundation. I understand and agree to comply with the terms and conditions outlined by the organization.
Terms and Conditions:
- I have properly registered and provided accurate personal information before attending the training(s) / competition(s). I understand that all confidential information provided by me hereto shall be used by Sittruli solely for the purposes of rendering services pursuant to this Agreement and, except as may be required in carrying out the terms of this Agreement, shall not be disclosed to any third party without the prior consent of myself.
- I take full responsibility to consult with my medical practitioner in case of my medical condition or fitness to engage in the training(s) / competition(s). I understand and agree to disclose any medical conditions and/or prescriptions to Sittruli Foundation prior to engaging in the training(s) / competition(s). I further understand and take sole responsibility to ensure my ability to participate in the training(s) / competition(s), and I hereby waive any and all claims and release Sittruli Foundation from all liability in connection with determining whether I am medically able to participate in the training(s) / competition(s).
- I understand and adhere to the code of conduct that includes behavior, sportsmanship, and ethics both on and off the field. This includes treating instructors, fellow participants, and facility staff with respect, refraining from any abusive or inappropriate language or behaviour.
- I understand that I must bring my own accessories, including a towel, water bottle, and wear appropriate attire for the training(s) / competition(s) as instructed by the trainer. I understand that taking snacks inside the playground is strictly prohibited.
- I acknowledge that sports training(s) / competition(s) inherently involve risks, and by participating, I accept responsibility for any injuries or damages that may occur during the training(s) / competition(s). I understand that Sittruli Foundation takes no responsibility for injuries or damages other than providing first aid and arranging immediate medical assistance when required.
- I understand that I may be photographed or recorded during training(s) / competition(s) for promotional or informational purposes. I provide my consent to the use of my name, image, and likeness for promotional or media purposes by Sittruli Foundation or its affiliates.
- I understand that the training(s) / competition(s) are happening inside the school/hospital premises, and I should maintain a high degree of discipline inside the campus. I further understand that I should not use any other area inside the school/hospital other than the designated play area given for the training(s) / competition(s) purposes.
- I understand that I may be responsible for maintaining and returning the equipment and uniforms provided by Sittruli Foundation in good condition.
- I understand that I need not pay any training(s) / competition(s) fees; Sittruli Foundation covers all the expenses, including coaching, equipment, and other expenses.
- I authorize Sittruli Foundation to seek medical attention on my behalf in case of an emergency. I will be responsible for any medical expenses incurred.
- I understand that I may need to have insurance coverage at my own cost for any injuries that may occur during training(s) / competition(s).
- I shall take care of my transportation within and outside the training(s) / competition(s) area. I understand that Sittruli volunteers will be available to help me commute only from the parking area to the playground and to the restrooms.
- I understand that I must comply with all rules and regulations of Sittruli Foundation, as well as any relevant sports governing bodies.
- I understand that any violation of the rules and regulations set by Sittruli Foundation or other reasons deemed necessary may result in the termination of my participation in the training(s) / competition(s).

- பயிற்சி(கள்)/போட்டி(கள்) கட்டணம் எதுவும் நான் செலுத்த வேண்டியதில்லை என்பதை நான் புரிந்துகொள்கிறேன்; சித்ருளி அறக்கட்டளை பயிற்சி, உபகரணங்கள் மற்றும் பிற செலவுகள் உட்பட அனைத்து செலவுகளையும் உள்ளடக்கியது.
- அவசரநிலை ஏற்பட்டால் என் சார்பாக மருத்துவ சிகிச்சை பெற சிட்ருலி அறக்கட்டளைக்கு நான் அங்கீகாரம் வழங்குகிறேன். ஏற்படும் மருத்துவச் செலவுகளுக்கு நான் பொறுப்பாவேன்.
- பயிற்சி(கள்)/போட்டி(கள்) போது ஏற்படும் காயங்களுக்கு எனது சொந்த செலவில் காப்பீடு தேவைப்படலாம் என்பதை நான் புரிந்துகொள்கிறேன்.
- பயிற்சி (கள்) / போட்டி (கள்) பகுதிக்கு உள்ளேயும் வெளியேயும் எனது போக்குவரத்தை நான் கவனித்துக்கொள்வேன். பார்க்கிங் பகுதியிலிருந்து விளையாட்டு மைதானம் மற்றும் கழிவறைகளுக்கு மட்டுமே பயணிக்க சிட்ருளி தன்னார்வலர்கள் உதவுவார்கள் என்பதை நான் புரிந்துகொள்கிறேன்.
- சிட்ருலி அறக்கட்டளையின் அனைத்து விதிகள் மற்றும் ஒழுங்குமுறைகள் மற்றும் தொடர்புடைய விளையாட்டு நிர்வாக அமைப்புகளுக்கு நான் இணங்க வேண்டும் என்பதை நான் புரிந்துகொள்கிறேன்.
- சிட்ருலி அறக்கட்டளை நிர்ணயித்த விதிகள் மற்றும் ஒழுங்குமுறைகளை மீறுவது அல்லது அவசியமாகக் கருதப்படும் பிற காரணங்களால் பயிற்சி(கள்)/போட்டி(களில்) எனது பங்கேற்பு நிறுத்தப்படலாம் என்பதை நான் புரிந்துகொள்கிறேன்.
